This week’s community Health Nursing assignment will be in three parts:
- Write a short Community Health Nursing paper, 250–300 words, addressing each of the following bullet points.
- Identify the topic for your community education project. Consider your community needs.
- Discuss what your topic is and why you chose that topic related to your community needs.
- Select your site for the community education project.
- Discuss why you selected the specific site for your presentation related to your topic and community needs and note where and when (place, date) your presentation will take place. (If you are still working on solidifying the site, discuss your options and what your plans are to get this solidified to be a good fit for your topic and community needs).
**Note: In this course, in every module assignment or discussion, due to the nature of the assignments, you are permitted to use first person, such as I, we, me, us, etc. Please submit the paper above and the permission form into the dropbox- these should be submitted in two files in the dropbox.

Community Health Nursing Education Project Introduction
Topic Selection
The topic I have chosen for my community education project is Type 2 Diabetes Prevention and Management. This topic is highly relevant to my community because of the growing prevalence of diabetes and its complications, such as heart disease, kidney failure, and vision loss (Ali et al., 2022). Many individuals are unaware of the risk factors, lifestyle changes, and early interventions that can prevent the progression of this disease. By addressing this issue, the project aims to increase awareness, encourage healthier habits, and provide tools for effective self-management.
Rationale for the Topic
I selected this topic because diabetes disproportionately affects underserved populations within my community. Limited access to nutritious food, lack of health education, and sedentary lifestyles contribute to rising cases (Ceriello & Prattichizzo, 2021). Education on preventive strategies, such as balanced nutrition, regular exercise, and routine screenings, is crucial in reducing the burden of this chronic disease. Furthermore, this project supports the broader community need for empowerment in managing health and preventing long-term complications.
Site Selection for the Presentation
The site I plan to use for my presentation is the local community health center. This location is accessible, trusted, and frequently used by residents seeking medical care and education. Holding the presentation at the health center ensures that participants can also access related resources, such as screenings or referrals. I plan to deliver the session in the health center’s community room on October 15, 2025. If scheduling conflicts arise, an alternative option would be partnering with a local church or community hall, which also serve as trusted gathering spaces. This flexibility ensures the project remains aligned with community needs and engagement.
