Evidence-Based Practice Project Proposal
Having a detailed project implementation plan is crucial to the success of the project. A project implementation plan is a document that has various components such as the budget, timeline, and stakeholders needed to implement the project and the methodology in which the project will be implemented (Ross et al., 2018). The purpose of this evidence-based practice project proposal is to describe the project implementation plan that will be used to implement an obesity prevention and management program for community-dwelling obese African American adults.
Setting and Access to Potential Subjects
My quality improvement initiative is focused on African American adults with obesity. The setting of the project will be in a community setting where patients who are overweight or those with obesity will be considered for inclusion. Participants will be sourced from community-based organizations, including community health care centers, social halls, religious organizations, to list a few. Based on the nature of the project, a consent form will be given to the patients before participating to ensure all ethical and legal considerations are adhered to. Timeline

Quantitative research is usually used when making observations on events or situations that affect people. In this case weight of the participants will be measured before and after the intervention, then compared to see whether there have been any changes resulting from the implemented change. Similarly, a short questionnaire will be issued to the participants to rate their experience and perception of the EBP change. This will provide feedback on the effectiveness and sustainability of the intervention.
The advantage of using questionnaires is that they are easy to administer, are economical, and provide some element of convenience to the research participants (Jones et al., 2013). A sample of the questionnaire is attached in the appendix section.
Intervention
The selected evidence-based intervention involves a fitness program involving a 30-minute walk, at least thrice a week. Lewin’s change model will be used as a framework to guide the implementation of the intervention. The model involves three steps, that is; unfreeze, change, and refreeze (Harrison et al., 2021). The unfreezing phase consists of communicating the urgency and explaining to the stakeholders why the evidence-based change is needed.
This is done to prevent resistance to change. In this stage, training will be required to facilitate adherence to the stipulated lifestyle changes required to make the program a success. Some of the topical issues in the training program will include nutrition and the importance of physical activity in maintaining a healthy weight. The second phase is to implement the change, which involves the actual rolling out of the project and monitoring the participants for compliance.
The final part would be to encourage the participants to maintain and sustain the change, that is, to make the 30-minute walk a routine, and if possible, increase the intensity of the physical exercise for better outcomes.
Stakeholders
Stakeholders can be categorized into two that is internal and external stakeholders. Internal stakeholders who will be involved in the projects include the leadership of the healthcare facility, nutritionists, physical fitness experts, and nurses. On the other hand, external stakeholders will consist of community-based organizations, families of participants, government agencies, such as the Department of Health, Illinois, to list a few. Stakeholders play a crucial role in ensuring the success and continued sustainability of a project (Powell et al., 2019).
One of the roles of stakeholders is to provide the resources needed to implement the project. Stakeholders, more so, external stakeholders, will also facilitate the recruitment of participants into the study, given the potential impact the EBP will bring to the community. However, to ensure their support, consistent communication will be required to make them feel part of the project.
Barriers to Implementation
One cannot discount the fact that there are likely to be barriers associated with the project. One of the barriers I am likely to encounter is lack of resources for the project. Given the priority of the organizations and other stakeholders, inadequate availability of resources could be one of the issues the project is likely to face, more so, human resources. Another challenge is low compliance with the stipulated project guidelines by the participants due to undervaluing the importance of the project. The last challenge could be technical challenges, for example, the inability to properly use the remote monitoring devices and data collection by the participants.
To deal with these challenges, the following measures will be proposed; the first measure is to train the participant on the need and importance of the EBP on their health outcomes. Another training program will be carried out on compliance and how to properly use the gadgets when going for walks for effective data collection. The issue of resources will be dealt with by elaborating to the management on the need to implement the project as part of the strategic interventions towards achieving the mission of the facility.
Feasibility of the Implementation Plan
The implementation plan is based on different evidence-based aspects that make it feasible to execute the planned change.
